FOR AGED CARE & PALLIATIVE CARE TEAMS

A gentle resource for the families in your care.

With Love Martha has designed thoughtful Support Kits for aged care and palliative care teams — each one built around a set of Your Gentle Guide cards, and made to sit gently beside the work your nurses already do.

THE QUIET CASE

The hardest conversation in healthcare — and the simplest tool to help.

86% of all exits from permanent residential aged care in Australia in 2023-24 were due to death (AIHW, Older Australians 2024). Every aged care facility in Australia is, in effect, an end-of-life care facility. Tools that help staff and families navigate that reality are not an add-on. They are core.

What’s inside

Every kit is hand-finished in Australia, with quiet attention to detail.

  • Your Gentle Guide — soft, supportive, plain-language cards with gentle ways to give and receive care, comfort, and presence — one set for the person nearing the end of their life, one for the loved ones walking alongside them.

  • Hand cream — for the small, tactile moments of care at the bedside.

  • Lip balm — a simple comfort when hydration becomes difficult.

  • Room spray — to soften a clinical space.

  • Notebook, pen and pencils — for letters, questions for the care team, or quiet reflection.

Three reasons facilities say yes

It helps families know how to be.

At the bedside, even the most loving family can feel lost — uncertain how to offer comfort, what to say, or simply how to be present in the hours that follow. That is not a clinical gap. It is a human one, and it often goes unmet. The Support Kit gives families gentle structure: things to do with their hands, words to write, a way to be present without needing to find the right thing to say.

It lifts perceived quality of care

The Support Kit is a cost-efficient way a facility can differentiate on compassion — the quality that families remember, mention in testimonials, and reference in accreditation feedback. The Support Kit is a visible, documentable expression of person-centred, dignity-led care.

It supports your staff as much as your families

Palliative care nurses, social workers and chaplains carry enormous emotional weight. The Support Kit gives staff something to offer families in moments when words are insufficient — reducing the emotional labour of navigating those conversations, and staying with the family long after the clinical team has left the room.

Wholesale pricing

Support Kits are available at wholesale rates for facilities ordering in volume. Pricing is tiered by order size. To discuss what works for your organisation, please get in touch.

Branding

Make it yours

We offer branding options for facilities who would like the Support Kit to carry their name — so that when a family receives it, they know it is a gift from your team. Branding includes insert cards, as well as custom packaging. To discuss what works for your organisation, please get in touch.

A Pilot Programme

A quiet way to begin.

For facilities who would like to experience the Support Kit before committing to a full order, we offer a structured pilot programme. A small number of kits, placed with families over six to eight weeks. At the end of the pilot, we ask for a brief conversation with whoever is closest to it — your palliative care nurse or social worker — to understand what families experienced and whether the Kit earned its place.

Pilot kits are available at a subsidised rate. If the pilot confirms what we believe it will, we move to a formal supply arrangement from there.

To enquire about a pilot, please get in touch. We work with a small number of pilot partners at any one time.
